Evio collects and processes personal information for Evio’s internal business purposes and for website-related purposes. Evio may collect information from the following sources: directly from you, automatically (such as through cookies or similar technologies on the website), or from Evio’s health plan or business associate customers.
a. Communications. You may choose to communicate with Evio through Evio’s website. If you provide your name, email address, organization’s name, or other information, Evio may use that information to respond to any communications, questions or inquiries made by you.
b. Job applicants. If you apply for a job at Evio, Evio mayuse your information as described below under “JOB APPLICANT INFORMATION”.
c. Social media. If you post information on a webpage Evio has established on a social media platform, we may use the information to respond or otherwise may use the information in the normal course of Evio’s business operations. We may collect personal identifiers (such as your social media username) and other personal characteristics that you have made publicly available on the social media website. Please note that the third-party operators of social media sites also receive such information, and their use of your personal information is governed by their own terms of use and privacy policies.
d. Aggregate information. Aggregate, non-identifiable information may be collected during your visit to the website, and we may use this information to analyze traffic data, resolve technical issues, and test and enhance website security and accessibility.
e. Automated data collection. Like many companies and websites, Evio uses Google Analytics to help Evio evaluate website traffic. Google Analytics may collect personal identifiers (such as IP addresses), internet or other network activity information, information about your devices (such as browser characteristics, device IDs, and operating system versions), and information about your visits to and usage of Evio’s websites (including the link you used to reach a given webpage).

f. Protected Health Information. As part of its business, Evio provides services as a business associate to health plans or asa subcontractor to business associates of health plans. In connection with these services, Evio may receive and use protected health information (“PHI”) as that term is defined under the U.S. Health Insurance Portability and Accountability Act, as amended(“HIPAA”). Evio uses PHI in accordance with HIPAA.
Evio uses this information to determine how many users have visited particular websites or have opened or viewed particular content. Evio may also use this information to improve website performance and accessibility, to enforce website Terms of Use, or to prevent malicious conduct.

Evio may disclose personal information for our business purposes described below.
a. Service Providers and Vendors. Evio may share information, including personal information, with vendors and service providers that process such information to perform services for us in connection with the websites or Evio’s other business operations.
b. Job applicants. If you apply for a job at Evio, Evio may disclose your information as described below under “3. JOB APPLICANT INFORMATION”.
c. Legal and regulatory disclosures. Evio utilizes a third-party platform, Greenhouse, to publish open job postings at Evio, and to collect job applications from potential candidates. Information aboutGreenhouse’s privacy practices can be found at: https://www.greenhouse.com/privacy-policy.
Evio may collect the following information during its job application and recruitment process:
- Your name, address, email address, telephone number, and contact information.
- Your resume, curriculum vitae, cover letter, work experience, education information, transcripts, or other information you provide to Evio as part of the job application and recruitment process.
- Interview or phone screening information.
- Details of the types of employment and roles you may be looking for, current or desired salary, benefits, and other terms of employment, and other job preferences.
- Sensitive and/or demographic information that you may voluntarily provide, or that is required to be collected as part of the recruitment process – for example, gender, information about your authority to work in the United States, medical and/or health information or disability condition.
- Reference information, including information provided bythird party references.
- Information related to any assessment you may take aspart of the interview screening process.
- If you receive a contingent offer of employment:
o Information received from background checks or drugtesting, including information provided by credit reporting agencies or other third parties.
o Information related to government identification (e.g. social security number, identification cards, etc.) and date of birth.
Evio will use this information for the purpose of carrying out its application and recruitment process, including:
- Communicating with you about the application and recruitment process.
- Assessing your skills, qualifications, and interests against available job opportunities at Evio.
- Verifying your information and carrying out reference checks and/or conducting background checks if you are offered a job.
- Creating and submitting reports required under applicable law.
- Complying with applicable laws, legal processes, or enforceable governmental requests or orders.
- Protecting the rights and property of Evio, its applicants, candidates, employees or the public as permitted or required by applicable law.
If you are offered and accept employment with Evio, the information gathered during the application and recruitment process will become a part of your employment record and will be processed according to Evio’s Employee Handbook and other human resources policies and procedures.
When required bylaw, Evio may seek your consent to process your personal information or to request information about you from third parties (e.g. educational institutions, credit reporting agencies, drug testing results, etc.).
Evio may use third-party service providers acting on Evio’s behalf to perform some of the services described above, including for the purposes of conducting background checks. Evio may also be required to disclose your information to governmental agencies or other third parties, such as tax authorities, courts, tribunals, or other law enforcement agencies or in response to legal process.
Evio may retain your job application materials for a limited period of time for purposes of:
- Considering you for other current or future roles at Evio.
- Complying with applicable law, including any laws related to the retention of information.
- Defending itself, its owners, directors, officers, employees and affiliates from legal claims.
If you do not want Evio to retain your job applicant information or would like for Evio to update your information for consideration for other job roles, please contact hello@evio.com. Please note, however, that we may be required to retain some information as required by law or otherwise as necessary to protect Evio from legal claims.
Certain jurisdictions have specific legal requirements and grant privacy rights with respect to personal information. Evio will comply with restrictions and any requests you submit as may be required under applicable law. Evio will respond to your request within the time period required by applicable law. However, Evio may not always be able or required to comply with your request, and Evio will notify you in such event.

This website is not intended for use by children. By using this website you affirm that you are 18years of age or older.
For the avoidance of doubt, Evio does not knowingly collect, maintain or use personal information from children under 16 years of age. If Evio believes that any information has been submitted by a child under 16 years of age, Evio will promptly delete such information.

This section applies only to personal information of consumers who are California residents.
Consistent with the California ConsumerPrivacy Act, as amended by the California Privacy Rights Act (collectively, the"CCPA"), Evio will honor a California consumer’s request:
a. for additional information about Evio’s data collection, use, disclosure, and sharing practices in connection with the consumer’s personal information;
b. for the specific personal information collected about the consumer during the previous 12 months;
c. for the deletion of the personal information we have about the consumer;
d. to restrict certain processing of sensitive personal information;
e. or for correction of inaccurate information.
A California consumer will not be discriminated against for exercising the consumer’s California privacy rights.

In most cases, you will be required to provide your name and email address so that we can verify your request, and in some cases additional information may be required. Except as described in this Privacy Policy or provided for under applicable privacy laws, there is no charge for a California consumer to exercise their legal rights.
California consumers may use an authorized agent to make privacy rights requests. We require the authorized agent to provide us with proof of theCalifornia consumer’s written permission (for example, a power of attorney)that demonstrates authorization to submit a request for the California consumer. An authorized agent must follow the process described above to make a request, and we will also (a) require the authorized agent to verify the agent’s own identity and (b) confirm the agent’s authority with respect to theCalifornia consumer about whom the request was made.
We do not sell or share personal information, as those terms are used under the CCPA, except as set forth this Privacy Policy.
Except for information collection and processing as described previously, we do not collect any “sensitive personal information” as defined by the CCPA.
In addition, Evio will delete personal information if a California consumer appropriately invokes the right to delete as to such personal information and no permitted exception applies.
